When I fill out the reservation box on the home page of my site, it takes me to the summary reservation form here.... http://isleofpalmsoceanrentals.com/site2/reservation-form/

The problem is that it is not laid out how it is supposed to be. I see the screenshots in the documentation of how it should be laid out, but mine is not like that at all.

How do I get the layout of the Reservation form to be exactly how it is in the documentation?
